Breast engorgement
Overview
Breast engorgement (swelling): What to do when there’s too much milk
On the 3-5 day after delivery, many women feel that their breasts suddenly become hot, hard as a stone, and very painful. This condition, known as engorgement or swelling of the mammary glands, is a sign that your body has shifted from producing colostrum to mature milk, and it has "arrived" in large quantities. Although this is a physiological process, severe engorgement can be very uncomfortable and make feeding difficult.
Why does engorgement occur?
The main cause is a sudden increase in milk production that outpaces the baby's needs. This is accompanied by an increased blood and lymph flow to the mammary glands, causing tissue swelling. This is the so‑called primary (physiological) engorgement.
Secondary engorgement can occur at any stage of breastfeeding if milk is not effectively removed from the breasts. Possible causes include:
- Long gaps between feedings.
- Improper latch, preventing the baby from emptying the breast.
- Abrupt weaning.
How does engorgement differ from mastitis?
It is important to differentiate these two conditions. In engorgement:
- Both breasts are usually affected evenly.
- There is no high fever or flu‑like symptoms (though a mild temperature rise up to 38°C may occur).
- The condition usually improves after effective breast emptying.
If you experience severe pain, redness in one area, and a high temperature, it may be mastitis. Proper latching and frequent feedings are the best prevention for both engorgement and its complications.
